(Note: This schedule is for all programs with the exception of the departments within the College of Liberal Arts and Social Sciences.)

All applicants (students and faculty) must submit the original plus nine (9) copies of the CPHS application and any relevant informed consent documents. Ten (10) copies of any additional material (such as questionnaires, approval letter, etc) are also required. The appropriate number of copies must be attached to the original application at the time of submission. All applications will undergo a preliminary administrative review to determine if the application is complete and meets the basic requirements for compliance with CPHS policies and procedures. Any application that is out of compliance will be returned without being reviewed.

Applications that qualify for "exempt" status (see list on the web site) are reviewed administratively. The original application and all related materials must be submitted to the Compliance Specialist for review and approval. Applications for exempt status are reviewed twice a month.

IMPORTANT NOTE: Be sure the application truly qualifies for expedited or exempt classification. Any applications submitted incorrectly will be referred to the next appropriate committee meeting according to the submission deadlines listed above.
